OKAMA Conference Registration Forms

We want to welcome you to the OKAMA Fall 2012 Conference and Trade Show, Wednesday October 24, 2012, at the Oklahoma History Center in Oklahoma City. To register you may: 1. Download, print and fax in the registration form. Either mail in the payment or pay at the conference. 2. Scan and email the registration form and mail or pay at the conference. 3. Download, print and mail in the registration form and payment. If your company pays $2,500 in dues your participants attend free, however let us know who is coming so we can produce a name tag in advance and have an accurate count for meals. See the attached registration forms which can also be found on the OKAMA website at www.okama.org. Additional conference information can be found on the website.
