OKAMA Workers Compensation Survey

Dear OKAMA Member: Attached is a letter and a survey from OKAMA President Bob Hawley. OKAMA is conducting a feasibility study regarding Workers Compensation for our membership. This survey will be used to determine if there is enough need and interest among the membership to investigate and possibly implement a self-funded, EMS specific workers compensation program. Included in this 10 question survey are instructions on how and where to find all of the requested information needed to complete the form. Additionally, a form letter is attached for you to request 5 years of loss history from CompSource or any other carrier you may be using. The only way we will know if this type of program is a viable option for our members is through this survey; your participation is needed. We are striving for 100% participation from OKAMA members. We understand the information requested on this survey may be considered sensitive or confidential so we have elected to have the data sent to and compiled by The Bramlett Agency in Ardmore, Oklahoma. They will act as an independent third party ensuring strict confidentiality while compiling the information for the feasibility study. This information is also being distributed by mail.
