OKAMA's Stars of Life Nomination Forms

OKAMA's second annual Stars of Life celebration will be held Wednesday, March 30th at 6 p.m. at the Oklahoma History Center in Oklahoma City. Every OKAMA member-organization is urged to select one employee or EMS colleague to be recognized at the event as a "Star of Life". One of these "all-stars" will receive an all-expense paid trip to represent OKAMA at the American Ambulance Assoication's Stars of Life Event in Washington, DC, May 1-4, 2011. Attached you will find a nomination/RSVP form and a map to the history center. Also remember our Spring Conference will be held the day before on March 29th. Plan on attending both of these fine events! Pass the word to your firends and colleagues via your own websites, Facebook - or the old fashion way - the telephone.
